I was chatting with an acquaintance last night about job stuff, and I asked him, "What's the worst professional mistake you ever made?"

He's a psychologist, and his answer was "letting a patient out of the office in a homicidal rage, without alerting security."

It was a little humbling, since my answer was, "I forgot to assign a review of the new Jamie Foxx album, and it charted."

Of course, my mistake had more of a global impact, but still.

So, what would you say was the worst professional mistake of your career? What were the consequences, and how did you resolve it?
